GENERAL

The purpose of this e-template is to provide users with a printable copy of the current Annexure 1 form for recording and investigating workplace incidents.

This e-template is provided to all users free of charge.

WHO NEEDS TO USE THIS?

This e-template is intended for all Health and Safety representatives, Accident and Incident Investigators, persons or practitioners who are responsible for reporting and recording details of incidents and accidents that occur in the workplace. 

WHY IS THIS E-TEMPLATE IMPORTANT?

This e-template is important because it documents the details of any incidents which have occurred in the workplace. Whenever an incident occurs that is considered a Section 24 incident or which requires any type of medical treatment other than first aid, an Annexure 1 form must be filled in to document the details. This is a legal requirement and is referred to in the General Administrative Regulations (GAR), under section 9(1). Records of completed Annexure 1 forms must be kept on file for a minimum period of three (3) years.
